As we enter the open enrollment period for 2016, you need to know that the Healthcare Plans have changed, so you will need to actually re-enroll into the program for 2016. Please let us know if you have any problems with the forms. Please also be aware that the forms provided to you online meet the criteria of the Insurance Commissioner for each State, and these are the approved forms for each provider. Please read the instructions carefully as you complete the documents, Thank goodness we only have to do this once per year!!
