New Employee Health Coverage Options and Changes for Existing Employees
Just a friendly reminder to complete your group health insurance options. You have 60 days from your date of hire to enroll in order to have coverage on the first day of the month following 60 days of employment.
If you wish to decline coverage please complete the form noting you are declining the coverage. Remember that if you decline coverage now you will have to wait until open enrollment in January to enroll in the program (unless there is a change in your status, i.e. marriage, new baby, awarded full time).
This form can be found in two places for your ease of access. It has been placed at the end of the Policies and Procedures section which every employee has access to, feel free to go there and access the forms and mark the unit as completed to keep your “Green Bars” up to date. Also, you can find this form in the Health Benefits section of your employee tool box along with the enrollment forms and health plan options.
